Hair transplantation - a few words:
It is a known fact that 7 out of 10 men and 1 out of 5 women lose hair at a young age, this makes them lose confidence and makes them feel older. Once hair loss starts, they then are confused as to what need to be done, in a market flooded with lot of unapproved medications they invariably fall for the wrong medications, lose a lot of money, trying to grow hair, while answer lies in a simple solution of hair transplant surgery.

Hair loss needs to be graded and the severe ones benefit only from a transplant surgery. Unfortunately there are a lot of misinformation regarding this surgery in India . What is exiciting for my patients is with recent advances in administering local anaesthesia, microscopic dissection of hair follicles etc, hair transplant can be made as natural as possible undetectable to others. I have now started robotic follicular unit hair extraction as an extension, this procedure works well when the follicular units required is less. With follicular unit extraction hair transplant has been simplified further with less downtime.

Causes of hair loss
Rather than going through the entire list of hair loss causes which people find it difficult to understand, there are a lot of diseases that can induce hair loss, including thyroid disease, auto immunity, fever, medications, dieting, stress etc. Aging can cause hair loss which can affect the entire scalp with thinning being detected after the age of 50 years.
The most commonest type of hair loss is androgenetic alopecia which affects males and females, with hair loss affecting the upper scalp, all though a lot of factors are blamed for this like using helmets, shampoos, poor circulation, none of this is true, and our Indian tendency of shaving the hair does not make hair growth better. Male pattern hair loss is a combination of faulty genes and hormones (dihydro testosterone-DHT) the genes are inherited from the genes are inherited from parents, and hair loss starts after puberty.The classical case of androgenetic baldness starts with thinning of hairs followed by eventual loss of hair itself, hairs on the scalp that is genetically affected by DHT under go this procedure. This can occur as a continous process or episodes of rapid hair loss, and it usually affects hair at the top of scalp, hairs at the sides and back are not affected by DHT and they form the donor area for hair transplant surgery. The best part of hair loss is that it can be treated when it is in the early stages with a tablet called finasteride and minoxidil lotions and more advanced cases with hair transplant surgery. If people understand this issue then it makes their hair loss problem a lot easier to handle. Lasers do not work on androgenetic pattern hair loss.


Follicular unit hair transplant can deliver natural life long hair, the reason being hairs are normally present in the form of follicles in the scalp, which may be one, two or three. when we do a hair transplant we maintain the integrity of these follicles, as they are cut from the donor strip, under high powered microscopes, hair growth has been proven to deliver better results when intact follicles are transplanted, where as dividing a follicular unit can hamper hair growth, hence it is always better to choose a doctor who uses microscopic dissection for hair follicles, since this takes in a lot of experience to do so not many hair transplant surgeons opt for this.

Follicular unit hair transplant involves removing donor hair from the back portion of the scalp , under local anaesthesia and sedation, cutting individual hair follicles under the operating microscope and then making tiny cuts less than 1 mm in size to transfer these follicles into bald areas of scalp.

The use of follicular units result in creation of natural, feathered hairlines with out the abrupt ending of hairlines caused by older techniques. The requirement of number of grafts vary it may be as less as 500 to as much as 3000 plus depending upon the severity of hair loss, patients require between 1-2 sittings depending upon the extent of hair loss, to achieve satisfactory density (what we mean by density is the number of follicles I one square centimeter of skin). We have seen in our practice in my clinic an average density of 60 plus follicles / square centimeter in the donor region, Following transplantation small crusts develop which will shed of subsequently, there is no bandaging involved, grafted hair commences growth in 6-15 weeks and grow at a rate of 1 cm a month. The success rate of transplanted hair is very high and they do not get affected by DHT due to their generic immunity from DHT hormone. To achieve natural results the experience of doctor counts because it needs artistic ability to achive placement and angulation of grafts to promote natural results, hence patients should look at the background of doctors before commiting to hair transplant surgery. Follicular unit hair transplant surgery also is useful for covering up scalp scarring from trauma, burns or older surgeries on scalp.

Robotic Follicular unit extraction
We have pioneered robotic follicular unit extraction technique in India Where in a robotic device extracts individual hair follicles, from the back portion of scalp, up to 700-1000 follicles can be harvested in a single sitting and transplanted.

The advantages of follicular unit extraction are
Avoiding the need to suture the donor region, a wider donor region is available for hair extraction, post operative recovery is a lot better and faster compared to conventional hair transplant surgery, however we now use this technique to treat clients whose requirement is less than 1000 follicles, or in clients who refuse the traditional hair transplant surgery. The use of robotic follicular unit extraction has made hair transplant easier in patients in whom the requirement is small, this device cuts and removes the hair follicle at the same time, hence the time required to extract large number of hair follicles is less, compared to punches which other hair transplant surgeons use.

We charge here around 30-40 rupees for each follicle that we transplant and we can transplant upto 2000 follicular units in one sitting.
Patients kindly note 2000 follicles (does not mean 2000 hairs) it may translate to 3500-4000 plus hairs.


Is hair transplantation successful?
Yes! Hair transplantation has been continually refined over the last 40 years and Australia has a proud record as a leader in developing and refining advances in this field. In experienced hands, approximately 90% of the transplanted hairs will grow in their new position, permanently. Hair transplantation does not achieve your original hair density but generally provides good to excellent coverage.

Is hair transplantation safe?
Yes. Hair transplantation has a justified reputation as being a very safe solution. Only a few patients experience minor side-effects such as initial swelling and temporary numbness in the transplanted area. Minor bruising is very uncommon. Complications are very unusual and may include minor infection at the graft site which easily responds to antibiotics.

Is the procedure painful?
No. The procedure is carried out under local anesthetic and mild sedation (which makes you drowsy but awake) so that no discomfort is felt during the procedure. Mild analgesics are provided afterwards to relieve any minor discomfort felt over the 24-48 hours following the procedure. Ice packs may be used for the first 48 hours after the procedure to minimise any swelling that may occur.

Are there any scars?
A minor scar is created in the lower scalp donor area after removal of the donor skin. This area is stitched closed with fine dissolving sutures and a new closure technique encourages hair to grow through the scar. In any event the surrounding hair covers this narrow scar easily. Even your hairdresser will have trouble finding it! The graft sites in the upper scalp heal with almost no scarring which may rarely be barely visible upon close inspection if seen at all. In any case it is effectively covered by the transplanted hairs.

How many procedures are required?
This depends on the size of the area to be treated and the density required. A higher graft number per session will reduce the number of procedures. It will be discussed with you at the initial consultation. It may be as little as two procedures.

When does the new hair appear?
After the procedure small scabs or crusts form on the grafts. These protect the grafts. After 7-10 days these will flake off by themselves or may be gently removed. At this
time, some of the transplanted hair seen above the scalp will initially be shed. This is normal. However the hair roots or follicles below the skin will produce new hairs after 8-12 weeks. Each graft generally produces between 1 to 4 hairs. At first only a few hairs are seen, but many hairs are now growing beneath the skin and they will appear gradually during the first six months. The results are at first gradual and subtle, but very certain. 8-12 months after the procedure, when the hair has had a chance to grow out, the results are generally very pleasing.

When should I start treatment?
Patients may have hair restoration surgery at any age. If your hair is thinning, it is better to start before you are completely bald so that you can use existing hair to camouflage the procedures. However, because hair loss tends to be both gradual and progressive, it is often unwise to start surgical treatment in a patient who is very young. Medical treatments such as Finasteride and Minoxidil may be offered to men with mild to moderate hair loss to help preserve the hair in the crown. Hair transplants can be used to fill-in the front hairline and thicken the front half of the scalp, and medical treatments can be used to maintain hair behind the transplants and possibly enhance the long-term.

FUE hair transplant is a surgical procedure in which skin containing hair follicular unit (each unit has 1-5 hairs) are extracted from one part of the body (the donor side) to implant to bald area (the recipient side). Follicular Unit Hair Transplant

FUT involves the removal of a strip of superficial layers of hair bearing scalp from the back and sides of the head (donor area). The strip is dissected into fine follicular units with the help of stereo microscopes and are transplanted on to the bald area (recipient area).

The main advantage of this method is to cover wider areas as it gives scope for obtaining more no. of grafts in a single session. If the donor area permits, one can transplant even up to 5000 follicular units (>8000 follicles) in a single session. Centers that use stereo microscopes and mantis microscopes and also incorporate four hook method of dissection (open method) will have lesser chance of graft wastage (transection).

Although this method leaves a scar in the donor area, by virtue of the latest advancements in the technology, this can be minimized by a technique called tricophytic closure. Many Indians by nature tend to develop little thicker & wider scars when compared to western people. Dr.Rajnish Anand has a developed a technique of suturing by which he tries to minimise the scar as small & as invisible as possible. This method helps in minimizing the scar which is almost invisible after some time and even new hair growth is seen through it in most of the patients. scar picture

The survival rate of grafts is very important to achieve good results post hair transplantation. We at Dr. Rajnish Anands take care in each and every step of the procedure like giving micro slits for implantation, adapting non-touch technique method of implantation, maintain low temperatures and implanting at good speeds resulting in drastically increased survival rate of grafts when compared to the older methods.

Only centres, that have more no of highly skilled surgical assistants with sufficient microscopes can carry out this procedure. This method is the time tested one, less expensive and gold standard in hair restoration.
Follicular Unit Extraction (FUE)/(DHI)/ FUSS/ etc techniques

It is not a solution for rectifying wide bald areas like Norwood VI or even IV. It is a good option for rectifying or covering smaller areas.

This method involves extracting single follicular units from the back portion of the scalp (donor area) and transplanting them on to bald area (recipient area). These follicles should be extracted from the permanent hair bearing zone only. Follicles which are extracted very near to the crown or nape of neck may go into baldness if the donor area is still susceptible for balding. 

We follow the latest method called power assisted Follicular unit Extraction. As we use very small sized punches the wound heals without much disfigurement. But in FUT method (strip method) we not only use the latest method called Trichophytic closure we modified the technique of closure and developed further so as to give you almost insignificant scar. Our modified method is called as Triplephytic closure. Triplephytic closure is a combination of Trichophytic closure & triple layered closure.

In general, FUT method is the most preferred method worldwide as it yields more number of grafts which helps to cover wider bald area in a session. Dr.Rajnish Anand has been practicing the advance techniques in hair restoration and is achieving very good results by FUT method.

Most of the transplanted patients are getting more benefited with FUT method in terms of results and coverage and also cost and it is least tedious to the patient and to the doctor also, hence Dr.Rajnish Anand is now more into FUT as it is giving good results when compared to FUE where one can achieve less number of grafts hence less coverage in a session.   

Long Hair Transplantation

With this revolutionary approach one need not wait for 8 to 12 months to see the results, un like in routine Hair Transplantation surgery.Here the results are instant and one can appreciate ones approximate picture with full length of hair immediately after the procedure.   Most of the Transplanted hair falls of 3-8 weeks after the surgery only to regrow after sometime; hence one has to wait a minimum of 8 months to appreciate the permanant re-growth in the implanted region.

  • Capacity of HT surgeon to use MICRO-BLADES to make the micro-incisions of sizes varying between 0.7-0.9 mm should be verified, as they not only determine the aesthetic outcome of the surgery but also helps to perform "Dense Packing" (i.e., implanting more than 40 units per sq. cm) .
  • The centre should be equipped with doctors capable of performing Tricophytic closure (advanced method of suturing) which helps in minimizing the scar on the donor area.
  • Centers should also be equipped to carry out the procedure, with minimal or negligible pain and with remote / no chance of post-operative complications like facial edema, pain or infection following the procedure, by practicing absolute sterilization techniques.

Quality Hair Transplant Surgeon

The Quality of a hair transplant surgery can be easily assessed by any one after full re-growth.

One has to look for the following aspects to assess the quality of the hair transplant:

Note: Visible growth of the transplanted hair can be seen only after 8 months following the surgery. How ever, It takes almost 15-18 months to achieve the density of the existing hair. Hair Weaving Baldness

Baldness becomes more obvious when scalp is visible through the hair. Camouflaging is the simplest and easiest way of dealing with baldness, thus giving out the illusion of thick hair.

Many people with hair loss and baldness tend to prefer hair pieces/wigs as they appear easy to comply.

Many people with diffuse hair loss prefer hair pieces/wigs to scalp as it is easier.

Wig hair is made from either synthetic acrylic fibers or natural hair. Wigs are of two types.
  • Interwoven
  • Worn-over
Interwoven wigs tend to rise up as the existing hair grows and hence require frequent adjustments.

Though modern wigs provide immediate coverage which looks natural, they may become uncomfortably hot during summer reducing patient compliance and are usually associated with numerous problems like itching, dandruff, detectability and maintenance.

These pieces of wigs are arranged by different methods like:
  • Hair weaving
  • Hair bonding
  • Hair clipping
  • Hair gumming
All these methods have their own disadvantages and therefore cannot be worn for lifetime.

Though all these methods are swift and less expensive they are temporary in nature and can never match the process of hair transplantation which is gold standard in hair restoration and much more cost effective in long run.

In recent times, newer technologies like spraying artificial keratin fibers have come in vogue to improve cosmetic appeal by masking sparse and thinned out hair.

Humans throughout history have dealt with hair loss in varying ways. Hair loss treatments have become more sophisticated in recent years, but over 70 million Americans are still balding. Scientists have, however, recently confirmed that a certain version of the twentieth chromosome is responsible for hair loss, so there may be hope for the future.

Wigs: The first solution

Covering up ones hair loss is a tried and true solution that has been in use for centuries. As early as 3100 BC, both male and female Egyptians were making the cover-ups from vegetable fibers, human hair and sheeps wool. Wigs later became popular around 2900 among the upper class in Sumeria, and evidence shows that the Romans were fond of both the half wig, known as the galerus, and the full wig, or capillamentum. Julius Caesar decided to forgo an imitation head of hair, instead opting for a comb forward.

Modern Cures

Wigs remained the only real solution to hair loss until the early 19th century when snake oil salesmen started touting the benefits of hair loss cures. Oils and formulas were the standard for over 100 years, until the Evans Vacuum Cap Company developed a scalp-exercising device in 1905 that purportedly stimulated circulation and hair growth.

Balding people had more options than ever before starting in 1980 with the advent of hair plugs,
a surgical procedure that moved individual hairs to balding areas. Eight years later, Rogaine hit the market, giving hope to balding people everywhere with its promise to stimulate hair growth without surgery. The Hair Club made a similar promise in 1991.

The traditional wig solution was revamped and modernized in 1995 with snap on hair that stayed in place and looked natural. That same year, micrograft hair solutions revolutionized hair plug technology.

A new drug treatment, Propecia, was released in 1998 to treat male pattern hair loss. The successful treatment lasted only as long as consumers took the drug but showed effectiveness, especially in the crown area.

In the 2000s, balding men and women could choose from even more hair loss solutions, including follicular unit extraction and automated hair transplant surgeries. The next step in treatment will be hair regrowth injection therapy, but scientists are still perfecting the technique.
